Yep. Your AGP card memory range  is indeed missing.

The nVidia driver -used- to use MTRR and set them by hand using the BIOS
configuration.

This looks like a BIOS problem to me.
Please post your BIOS' AGP configuration. (Aperture size, caching, AGP
speed)

Second thing (might be unrelated): please post the output of glxinfo /
glxgears.
