In my experience the only thing likely to cause serious problems are new kernels, so I exclude=kernel* for automatic updates.
There are several packages that can cause global problems. They include grub (maybe won't boot) glibc selinux initialisation scripts bind (in some cases) dhcp client (can prevent network starting)
But surely other problems are going to show up at some point, whether you do it automatically or not? You cannot tell beforehand if installation of a package is going to cause difficulties. Or at least I can't.
If I've updated, say glibc, and then problems arise then there is some prospect I'll think of glibc as something that changed.
If I don't know something's updated there's a good chance I will think it wasn't.
