I get the same output on my machine when I insert a DVD+RW that did not
burn properly:
# dvd+rw-mediainfo /dev/hdg
INQUIRY:                [_NEC    ][DVD_RW ND-2500A ][2.36]
GET [CURRENT] CONFIGURATION:
:-( no media mounted, exiting...

When I insert this DVD, the drive churns for several seconds before
deciding that the disc is unreadable.  I keep the disc around in the
hope that I will run across a burner some time that will "fix" it.

I do not know of a command that will perform the equivalent of "format
this DVD+RW, even though you (the drive) think no DVD+RW is loaded."  I
tried several tools on Windowx XP and none of them could bring the disc
back to life, either.
