More information about your network would help. Is there a DHCP
server on the network eth1 is connected to? Also, is the cable
plugged in all the way on the card? I know it is a dumb question,
but it happens, so it pays to double check.

Also, are you replacing a card, or adding a card? If you are
replacing a card, you may have a line in ifcfg-eth1 that looks
something like "HWADDR=00:02:A5:F4:91:5E". If so, you will need to
remove it, or change it to match the new card. (Long shot - you
should not get to the Determining IP stage if it does not match.)
