I don't recall ever setting that from the cups admin gui. I think it is set in kde under the locale settings (not certain, though). I have three instances of my printer set up, with different configurations for different jobs. I'm pretty certain that the first time I used each of them in kprinter I did set the page size, then hit the Save button - still within kprinter - and all has been well since. In the past I have also used xpp quite a lot, and I think I did the same there. FWIW, I set my browser to print to kprinter (or xpp if you wish) which gives you access to whatever configuration you have set up as your default - in my case, the choice of all the printer definitions.
