Surely a box kept up to date would not have those security issues. Am I missing something?

Only if RH is:-)

It's still the default MTA installed by Red Hat's installer.

I don't use it myself, but that's to do with ease of configuration and the fact I was using Debian for a while, where sendmail was broken and unlikely to get fixed any time soon, so I switched to postfix which I like.

sendmail has an enormous share of the "market" - it can't be _that_ bad.
