Before starting the upgrade, did you do a "media check" to make sure all the CDs were correct?
You might want to retry the upgrade but specify "linux nocddma" at the boot prompt. That might work better for you.

It turns off DMA access for the CD drives. It'll be slower but it *might* alleviate the problem you're having.
