if you dont know much about what your doing, and aren't looking for a kernel update to fix a particular problem i wouldn't bother with it until bugs with the new kernel are ironed out.
this i thinl is true when there is a kernel major version jump eg 2.6.9 to 2.6.10. typically minor version jumps such as 2.6.9-xxx to 2.6.9-xxx+1 aren't that intrusive.

I usually use yum to do my kernel updates. So far I have never run into any problems... that being said... your mileage may vary.
the problem that arises during update is that the old kernel is removed. So when You do not reboot the machine, it might get into trouble when it e.g. tries do load a new kernel module after the update and all it finds in /lib/modules aer the new version's kernel-objects.
Therefore the default behaviour is to install the new kernel as a default, which (hopefully) manages to boot during the next startup, and leave removal of the old kernel to the user.
