Sometimes it would fail after the second or third reboot. Nothing seemed consistent or identifiable about what actually was happening. The final fix was to reset BIOS to a standard config. It appeared that having bios set for optimal performance under one OS caused it to be unstable under another OS/Version. I still have not identified exactly which setting was causing the instability, but after choosing the factory "optimal settings" the OS now is stable both during install and operation. YMMV but you may want to look at BIOS settings as a possibility.
