Just some more info when I insmod the driver at the
prompt:

[root@server root]# insmod 3w-xxxx
Using
/lib/modules/2.4.22-1.2197.nptlsmp/kernel/drivers/scsi/3w-xxxx.o

[root@server root]# fdisk -l /dev/sda

Disk /dev/sda: 750.1 GB, 750174732288 bytes

So it's coming up within the OS fine, the problem
seems to be from the kernel's bootup sequence, where
it detects the controller, assigns the device name,
shuts down the controller (effectively removing the
device name) and then continuing to boot.

Why would such a thing happen and how would I resolve
it?
