You didn't say what the hardware was, but several boxes of blank DVDs I've gotten lately came with a card warning that certain models of Pioneer DVD writers could be destroyed by writing DVD-RW/DVD+RW discs without a firmware upgrade. It sounded crazy to me, but I guess it happens. It's a long shot, but maybe your drives have a similar problem. I'd also check ventilation. These drives are known to get pretty hot when burning.
