Yes, a hard drive failure.

That reinstall was riddled with stuff that should have been put in but wasn't,
and I believe I made some comments about such over the next week or so, but
ntpd continued to run normally till the last week of Dec 2008.

Hummm, :

preinstall scriptlet (using /bin/sh):
/usr/sbin/groupadd -g 38 ntp  2> /dev/null || :
/usr/sbin/useradd -u 38 -g 38 -s /sbin/nologin -M -r -d /etc/ntp ntp
2>/dev/null || :
postinstall scriptlet (using /bin/sh):
/sbin/chkconfig --add ntpd
:
preuninstall scriptlet (using /bin/sh):
if [ "$1" -eq 0 ]; then
        /sbin/service ntpd stop &> /dev/null
        /sbin/chkconfig --del ntpd
fi
:
postuninstall scriptlet (using /bin/sh):
if [ "$1" -ge 1 ]; then
        /sbin/service ntpd condrestart &> /dev/null
fi

Which looks benign on the face of it. ???
