My /etc/fstab looks more like:

/dev/VolGroup00/LogVol00 /        ext3    defaults    1 1

My /etc/mtab looks like:

/dev/mapper/VolGroup00-LogVol00 / ext3 rw 0 0

There's no "on" or "type".  (Obviously there are more lines.)

My only other thoughts are to grep dmesg for "VolGroup" or "LVM" (not much
about them in my logs, probably because it is working), and to look at the
commands at the end of "man lvm", such as lvdisplay, vgdisplay,
lvmdiskscan, lvscan, pvscan, vgscan, and vgck.
