Your filesystem may be damaged. Your /disk/ may be damaged. Boot with something else (e.g., a bootable CD) and run fsck (e2fsck) on the disk. If that is not the issue, try reading the whole disk device (dd if=/dev/hd? of=/dev/null bs=1048576 count=1048576).
