I'm not going to repost your whole dmesg but I'll give you some comments on the "interesting" parts.

I get the identical output on my HP laptop right down to the repeated "bcm43xx: Radio turned off". The next lines from bcm43xx are where things diverge:

I'm having trouble with WEP authentication (still) but the corresponding dmesg section for me is:

On your box I'm guessing that the driver isn't finding the firmware or there is some problem with the firmware based on the:

bcm43xx: Error: Microcode "bcm43xx_microcode5.fw" not available or load failed.

message. Check the documentation that came with the fwcutter utility and see if you have the firmware in the correct directory, right permissions, etc.

P.S. BTW, the odd salutation ("Did you ever get an answer on this?") was due to me running across your posting on http://fcp.homelinux.org/ and me miss-reading the date.
