I found a linva bugzilla bug

and it confirms that the livna MadWiFi is broken with NetworkManager.

Subcribe to this bug to find out when livna MadWiFi will work with NetworkManager.

Is it possible for you to upgrade to FC5?
The version of NetworkManager in FC5 (0.6.x) works with madwifi fine for me. Possibly this version is in updates-testing, or you could grab the one in development and install it on FC4...would recommend upgrading instead of that though.
