that's really odd that it should crash on disk format...

if a disk passes media check...you gotta believe that it's good.

you might want to run memtest just to rule out the issue.

I imagine you could use an alternate console like I described before
'after booting' before going in to disk druid, and run fdisk, mkfs to
create the partitions and the filesystems and just select the already
created partitions in disk druid but not format them again.

Your suggestion that RH 7.2 works suggests to me that you have some odd
type of disk controller (SCSI ?) that might not be supported any
longer...do you know what drive controller you have in that thing?

It does seem to be some type of hardware problem.
