*They* need to use some option to avoid downloading the same message
again in their mail client, and your mail system needs to use the same
unique IDs with the same messages (which is most-likely does).

POP3 doesn't have a way for the server to know what's
new/old/read/unread, for it to hold back old messages and offer new ones
to the client.  It just provides a list of what's on the server for the
client to do whatever it wants to.
