ln -s /dev/ttyS4 /dev/modem
To make that surviving udev you will have to create a custom udev rule
in /etc/udev/rules.d/10-custum.rules, i.e. like:
BUS="serial", SYSFS{product}="USR modem", KERNEL="ttyS4",
NAME{ignore_remove}="modem", MODE="666"
