Sendmail can handle user@domain "things". Depends on about what you are speaking. If user@domain is a username you are speaking about authentication. That is handled by SASL and is a matter of the authentication backend/user auth data storage. If it shall be mailbox name, then you speak about mail storage and that is not any topic of Sendmail but the MDA/LDA and the POP3/IMAP server.
