Answered my own question. These are the settings that worked for me:

	ModelName "SGI 1600SW FlatPanel"
	HorizSync 31.5 - 121.0

EndSection

Section "Device"
	Identifier "SGIVideo"
	Driver "i128"
	VendorName "Videocard vendor"
BoardName "Number Nine Revolution IV (T2R4)"
EndSection

Section "Screen" ~ Identifier "SGIScreen" ~ Device "SGIVideo" ~ Monitor "SGIMonitor" ~ DefaultDepth 24 ~ DefaultFbBpp 32 ~ SubSection "Display" ~ Depth 24 ~ FbBpp 32 ~ Modes "1600x1024d32" ~ EndSubSection EndSection - -- Adam Lanier Bernard L.
