Fedora kernel includes some patch that vanilla kernel hasn't.
The rpms are made from original tarball and a special file (called specfile) that includes all informations (dependencies, descriptions, patch ecc...): there's a program (called rpmbuild) that processes the specfile, patches the sources, compiles it and then builds the rpm.
