Default icons on control panel an desktop changed for unknown reason

Sometimes when booted into the desktop, the red hat icon on control 
panel replaced by a big foot print, and icon of
blank papers replaced the original Computer and Start Here on desktop, 
any one has any clue how to fix this ? It
didn't happen everytime but sometimes.
