Seems like it could be a connection defect. You might try reseating
the modem in its slot, clean the contacts while you have it out and
maybe try it in a different slot.

Another point.... over the years I've come to understand the
advantages in leaving a computer turned on all the time, if it is used
daily. Turning one off and back on each day can caused "chip creep" as
parts warm up and cool down repeatedly.
