I co-wrote something called issue-tracker (www.issue-tracker.com) it
runs on either Postgres or MySQL (it's been ported to Oracle, but I
don't think we host that version at the sourceforge site). It's coded in
php. 

Red Hat uses it for high touch enterprise support contracts. It has a
gui interface, but you can interact via e-mail (eg you can get an e-mail
on your pda and respond, assign, etc). It can send an SMS page as well.
Lotsa good stuff you'd want in a ticket tracking system.
