After my frv patch, nobody uses flush_tlb_pgtables anymore, this patch
removes all remaining traces of it from all archs.

This is part of a couple of pre-reqs cleanup for my mmu_gather work,
which makes things easier later on. I'll do more cleanup afterward, such
as removing flush_tlb_all() which isn't used anymore, etc...
