Remove all traces of "raw devices" support.

since this feature is listed as being obsolete since 2.6.3 and was
scheduled for removal back in Dec of 2005, I figure there wasn't much
harm in getting rid of it.
i'm trying a "make allyesconfig" build as we speak -- so far, it's
moving along nicely.
