[git-or-PATCH] IRQ handler cleanups

This is the pure-cleanup stuff I pulled out of this morning's
remove-irq-arg work.  No behavior changes, just killing dead code,
killing unused 'irq' arguments in sub-functions, and killing void*
casts.

Please pull from 'irqclean-submit1' branch of
