The lockup occurred in the network code it looks like?

The report from dpt_i2o indicates that a command took longer than
expected, but was apparently dealt with. Could the extended response
time to storage tickle the network code or the soft lockup detection?

The fix may very well be to ensure that the scsi bus is stable. Cabling,
termination, drive firmware, card firmware, and drive status (age?) are
all in tip top condition. Would not hurt to call Adaptec Technical
support to ensure that you follow a cookbook checkout of the scsi bus
components.
