[PATCH 4/5] dasd: backout dasd_eer module

The module went in after it was resent without addressing any issue.
It still has a questionable ioctl interface, is split bogusly out of
dasd_mod, uses a non-typesafe notifier chain mechanisms, etc.  Back it
out for now, designing a sane interface for this needs some time.  I'll
do my best to help the IBM people to get there.
