I recently did some experimenting with my T42 (2378) and I found that an
external projector works reliably at various resolutions if you simply
boot the Thinkpad with the projector connected to the external VGA port.
If your VGA port is not live then you may need to go and change the BIOS
settings for the default LCD screen and VGA to be live.

I found that the Fn-F7 key works correctly until the X server is loaded
- In other words the Fn-F7 toggled between LCD -> VGA -> LCD/VGA as
expected until X is loaded. After X is loaded the Fn-F7 key stops
working as Matthew descibes above.

Good luck.
