These teo commands run sequensially (in a script) are the best way to format a dos disk: fdformat /dev/fd0H1440 mformat -t 80 -h 2 -s 18 a:/

I've used fdformat lots of times. I've never used mformat.

It's just about possible that I've always had floppy images that I wanted to copy to a floppy and only ever used fdformat to clean and test floppies to find one that works.
