I use dovecot to serve up IMAP. My maildir folders look like this:

~/.maildir/.INBOX
~/.maildir/.INBOX.fedora
~/.maildir/.INBOX.razor-users
~/.maildir/.spam

In my mail client, the folders appear like this:

INBOX
   fedora
   razor-users
spam
