You can increase the Sendmail and the milter log levels to get a more
verbose output what's going on in that cases:

define(`confLOG_LEVEL', `15')dnl
define(`confMILTER_LOG_LEVEL', `15')dnl

both in sendmail.mc.

As the above log message comes from Sendmail I would guess the problem
is on the Sendmail side.
