One way this can happen is if you forgot to umount it.  Automount (aka autofs) 
is a cool service that monitors the mountpoint (ie, /mnt/flash or whatever) 
and dynamically mounts the associated device on access.  Then a configurable 
period after the last access, eg, 2 seconds, it automatically and cleanly 
unmounts the device.  usbautomount (see the URL in my sig) uses this to make 
USB devices accesses a lot simpler.

There is a devlabel service as well that can make available /dev/sdn at a 
fixed alias depending on what it is, but I didn't look into it yet.
