Happened to me yesterday.

I had opened a terminal, and was in /mnt/cdrom as my normal user then su'd
to root to do something. I was NOT in the crom dir as root so I forgot i
was still in it as my normal user.

if that doesnt work, try fuser -k /dev/cdrom
