Here's a version that gets rid of a lot of confusing backslashes:

/^\(\s\)*#endif/ {
s,/\*,//,
s,\*/,,
}

\,^\(\s\)*/\*.*\*/\(\s\)*$, {
s,/\*,///,
s,\*/,,
}

\,^.*/\*.*\*/\(\s\)*$, {
s,/\*,///<,
s,\*/,,
}

s,^\(\s\)*/\*,/\*\*\n,

s,^.*\*/,\n\*/,
