In CONFIG_DISCONTIGMEM case, CONFIG_VIRTUAL_MEM_MAP is selected.(looks always...)
ia64 uses private page_to_pfn() func when CONFIG_VIRTUAL_MEM_MAP.
Replacing only FLATMEM case.
